Ohio Vax-2-School Official Rules

To Enter

From Monday, October 4, 2021 at approximately 8:00 a.m. EDT (the "Promotional Period"), Eligible Ohioans may enter the Ohio Vax-2-School ("Promotion").

The drawings will be conducted on a future date as determined by the Ohio Department of Health and announced one week prior to that date, but no later than December 1, 2021.

Winner announcements will occur on a future date as determined by the Ohio Department of Health. The winners for the $10,000 scholarships will be announced via Twitter and Instagram by @ohiovax2school and posted on the ohiovax2school.com website after the announcement. The grand prize winners will be announced on the Lottery broadcast at approximately 7:29 p.m. EDT, on the day of award.

Entry must be completed through ohiovax2school.com 11:59:59 EDT on the date announced to participate in at least one of the drawings.

By successfully transmitting a completed online entry form, with the appropriate information as directed, each participant will be entered into the Promotion.

If you do not have access to the internet, you can still be entered into the drawing by contacting the Ohio Department of Health (“ODH”) call center, which is open seven (7) days a week from 9 a.m. to 8 p.m. EDT and can be reached at 1-833-4-ASK-ODH (1-833-427-5634). You must provide the requisite information including that you are the parent or guardian of a minor that is being registered for the drawing.

One entry per person. Eligible Ohioans can only submit one (1) entry into the drawings. Prior to the drawings being held the entry list will be reviewed, and all duplicate entries will be removed. A parent or guardian may submit up to one entry per minor.

All entries received will be included in all drawings for Ohio Vax-2-School, unless you win a daily drawing. All daily drawing winners will be removed from future drawings.

All information submitted online by entrants is subject to, and will be treated in a manner consistent with, the State of Ohio’s Privacy Policy, which is currently accessible at: https://ohio.gov/wps/portal/gov/site/help-center/privacy-notice-and-policies.

Eligibility and Verification

You/your child or person under guardianship are eligible to participate in the drawing if you meet all the following requirements:

  1. Are a living citizen of the United States and a permanent resident of State of Ohio.
  2. Are at least 12 years of age but not older than 25 on the last day of the drawing. Any young Ohioan age 5 to 11, may also be registered by their legal guardian if a COVID-19 vaccination is authorized for use in this age group by the FDA and administered prior to the registration deadline.
  3. Are not incarcerated (in prison or jail) for a felony conviction under the laws of this state, another state, or the United States.
  4. Is not in the custody of the Department of Youth Services.
  5. Is not an employee or Officer of the Ohio Lottery Commission, the Ohio Department of Health or the Ohio Governor's Office, or any blood relative or spouse of such an employee or officer living as a member of the employee's or officer's household.
  6. Has received at least your first dose of COVID-19 vaccine if receiving the Pfizer or Moderna vaccine or one dose if receiving the Johnson & Johnson vaccine. Ohioans ages 12 to 25 who participated or are currently participating in a clinical trial for other COVID-19 vaccines are also eligible. Ohioans 5 to 11 who participated in a clinical trial for other COVID-19 vaccines will also be eligible once a COVID-19 vaccination is authorized for use in this age group by the FDA and administered prior to the registration deadline.
  7. Has not won a prize in the Ohio Vax-A-Millions drawings.

All winner information and eligibility will be reviewed prior to the declaration of an Official Winner. Potential winners’ information will be validated by State of Ohio officials.

In the event that an entry is deemed invalid, a discrepancy is found, or the winner cannot be located based upon the contact information submitted, the prize will be forfeited.

In the event that the winner does not wish to receive a prize, refuses or fails to provide verification information, or does not promptly respond to the initial contact by State of Ohio officials, that winner may be deemed ineligible. An alternate winner may be announced at the discretion of ODH.

The promptness and degree of reasonableness of efforts to locate a winner or of any decision to deem a winner ineligible based upon these rules shall be at the sole discretion of the State of Ohio.

By entering the drawing, an individual or a parent or guardian of a minor child or person under a guardianship explicitly authorizes the Ohio Department of Health to access vaccination records or contact the provider submitted on the entry form to verify that a COVID-19 vaccination has been received.

The COVID-19 vaccination is not required to be obtained in the State of Ohio. However, proof and verification of vaccination, to the satisfaction of the State of Ohio, is still required. For Ohioans receiving either the Pfizer or Moderna vaccines, at least the first COVID-19 vaccination must be obtained prior to the entry deadline. Ohioans ages 5 to 25 who participated or are currently participating in a clinical trial for other COVID-19 vaccines are also eligible.

Verification of all information must be presented to claim any prize.

Internet Conduct and Limitations of Liability

Any attempt by a user, participant or any other individual to deliberately damage the promotion website or undermine the legitimate operation of this Promotion, is a violation of criminal and civil laws, and should such an attempt be made, the State of Ohio reserves the right to seek the prosecution of and/or damages from any such person or persons as permitted by law. Furthermore, such person may be excluded from the Promotion.

Neither the State of Ohio, nor the Governor of the State of Ohio, the Lieutenant Governor, any of its employees, commissioners, owners, directors, officers, affiliates, subsidiaries, contractors, assignees and agents are responsible for late, or incomplete or misdirected entries, or responsible for incorrect or inaccurate information, whether caused by the website, or by any of the equipment or programming associated with or utilized in this Promotion.

Neither the State of Ohio, nor the Governor of the State of Ohio, the Lieutenant Governor, any of its employees, commissioners, owners, directors, officers, affiliates, subsidiaries, contractors, assignees and agents are responsible for any error, omission, interruption, deletion, defect, delay in operation or transmission, communication failure, regardless of cause, with regard to any equipment, systems, networks, lines, satellites, servers, computers or providers utilized in any aspect of the operation of the Promotion, which may include without limitation, inaccessibility or unavailability of the internet, the website or any combination thereof; theft or destruction or unauthorized access of the website; any injury or damage to the participants or to any other person's computer(s) or smartphone related to or resulting from participating in this Promotion, or downloading materials from or use of the website.

Random Drawing and Results

The winners of the drawings will be randomly selected from among all eligible entries received.

Subject to verification of eligibility and compliance with these Official Rules, the potential winners drawn and verified will be declared the official winners of the Ohio Vax-2-School drawings.

Prizes

Daily Prizes
One hundred fifty (150) $10,000 scholarships will be awarded.

Grand Prizes
Five (5) $100,000 scholarships will be awarded.

General Rules

All prize details shall be determined in the sole and absolute discretion of the State of Ohio. Ohioans who do not meet the eligibility requirements defined in these official rules are not eligible to win.

Award funds will be distributed from the Ohio Department of Health to the Ohio Tuition Trust Authority, known as the Ohio CollegeAdvantage Direct 529 Savings Plan (the “Plan”). The Plan will accept the funds and place them into an account on behalf of each of the winners as directed by ODH. ODH will manage the funds for the recipients as beneficiaries. As structured, the account can be used for qualified tuition and related expenses including expenses associated with room and board, and payment of existing student loan debt. Winners will be notified of the specific permitted uses of the funds along with its accompanying terms and conditions by the ODH. Award funds may be put towards enrollment at any eligible college or university located or doing business in Ohio for a four-year program, two-year program, graduate program, community college, or technical/trade school. Winners are advised to contact a tax professional for advice specific to their situation.

The drawing prize may be reduced by monies owed to the Ohio Department of Job and Family Services, the Ohio Department of Taxation and/or or the Attorney General’s Office, if applicable.

Entry into the drawing by an individual or a parent or guardian of a minor child or person under a guardianship explicitly constitutes express permission for the State of Ohio and its agencies to announce the name of winner without notification, use the prize winner's name and/or likeness worldwide, in perpetuity, for purposes of advertising and trade without further compensation, notification or permission unless prohibited by law, in any and all media now known or hereafter devised.

Further, by accepting a prize, each prize winner agrees to hold the State of Ohio, including its Governor, Lieutenant Governor, any of its employees, commissioners, owners, directors, officers, affiliates, subsidiaries, contractors, assignees and agents, absolutely harmless against any and all claims and/or liability arising out of the ownership and/or use of a prize. Each prize winner assumes all liability for any injury or damage caused, or claimed to be caused, by participation in the Promotion or use or redemption of any prize. The State of Ohio makes no warranty, guarantee or representation of any kind concerning any prize (or any portion thereof) and disclaims any implied warranty. By participating in the Promotion, all entrants agree to be bound by these Official Rules.

All materials submitted become the property of the State of Ohio and will not be returned. Prizes are nontransferable to any other person, including relatives or friends. Fulfillment of the prize will be dependent upon winners completing and submitting all required paperwork by 5:00 p.m. on Friday, December 10, 2021 so that scholarship accounts can be set up prior to expiration of funding availability. Failure to complete and submit paperwork in a timely manner will result in forfeiture of award.

The Ohio Vax-2-School Official Rules and Drawing Procedures used by the Lottery are available for review and may be amended or revised at any time at the sole discretion of the Executive Director of the Lottery and the Director of the Department of Health. The drawings will be observed by representatives of the Auditor of State.

The State of Ohio, at its discretion, reserves the right to select and/or change the prize(s) to be awarded as part of this Promotion or end the Promotion prior to all prize winners being drawn.
