Children ages 12 to 17, who have received their first dose of the Pfizer vaccine, may be registered by their legal guardian beginning October 4, 2021.

Any young Ohioan age 5 to 11, may also be registered by their legal guardian if a COVID-19 vaccination is authorized for use in this age group by the FDA and administered prior to the registration deadline.

The entry deadline will be determined by the Ohio Department of Health on a future time and announced via Twitter and Instagram at @ohiovax2school.com.

Entrants will only have one valid entry in the drawing. Duplicate entries will be removed prior to the drawing.
